• Player Contract -Part of your Online Registration - Make sure you have digitally signed it during your Registration.
• Physical - Part of your Online Registration - Make sure you have digitally signed it during your Registration.    If you child has had a concussion - we would need a RELEASE from the Attending Doctor to clear for practice.
